I am purchasing admission to Long Island Family Premiere fundraising event (the “Event”) being hosted by United Jewish Appeal-Federation of Jewish Philanthropies of New York, Inc. (“UJA-Federation”) on September 15, 2020 (rain date September 16, 2020), at Old Westbury Hebrew Congregation. I understand that the Event is a live drive-in film at which attendees will be permitted to sit outside of their cars.

I have read and understand the list of precautions that UJA-Federation has put in place in an attempt to maintain social distancing during the Event. I understand that UJA-Federation will not be checking attendees’ temperatures, and that while UJA-Federation is encouraging attendees to wear masks during the Event, it will not be monitoring compliance with this request. I further understand that precautions being taken do not guarantee that the Covid-19 virus will not be transmitted to me or my guests during the Event.

I assume the risk of contracting Covid-19 from other individuals who may be present at the Event, including but not limited to guests, UJA-Federation staff and volunteers, vendors, entertainers, and venue staff. To the maximum extent permitted by law, I hereby waive, release, and forever discharge UJA-Federation and its officers, directors, and employees (collectively, the “Releasees”) from any and every claim arising from or by reason of the actual or alleged transmission of Covid-19 during the Event (“Claims”), whether or not based on the negligence of, or breach of contract by, any Releasee or any other party for whose acts or omissions any Releasee may be responsible. I agree to indemnify, hold harmless, and defend UJA-Federation from any claim brought by any Event attendee who gains admission to the Event by being present in the same car as me.



Occupants of each car will be assigned a designated area next to the car where they can sit to watch the show. Sitting areas will be arranged in a checkerboard fashion so that no two areas are adjacent.

Guests are asked to remain in their car or in their designated sitting area during the event.

Any guest who feels the need to leave their designated area must wear a face mask and practice social distancing. Guests are encouraged to wear masks during the entirety of the event.

A basket of packaged snacks will be delivered to each car by a waiter who is wearing a mask and gloves. No other foodservice will be available at the event.

Guests will be able use the bathrooms inside the synagogue; there will be a valet at synagogue entrance and at the bathroom doors to provide assistance.

Cars will not be permitted to enter the parking lot more than one hour before show time. Cars will not be permitted to enter the parking lot after the event has started.

To ensure everyone’s safety and avoid disruptions to other guests, we ask that you not leave the event until the program is over.

Guest are asked to leave the event promptly after the event has ended.
